Holly also shared a few notes of interests regarding her treatment of this patient using Postural Restoration™ techniques.
“Rhian presented 8 months s/p single screw fixation for a slipped capital femoral epiphysis with severely limited left hip PROM/AROM. The most significant was that she had -10 degrees of FAIR.  She had excessive left genu varus, left trunk side flexion, limited hip extension, calcaneal varus then valgus with mid stance to late mid stance for gait.  She also had an excessive left anterior rotated ilium. She was not able to flex her hip past 60 degrees. She was started in sidelying and hooklying positions for PRI activity that would assist with left AFIR incorporating left hamstring, left IC adductor, right glut max IR. We also used sitting on a high stool to get a left hemi bridge with pull downs (modified due to her ROM restrictions). Protonics® was incorprated as well in prone and supine to assist with reducing her anterior rotated ilium/hemilordosis. She has also worked a lot of on left IO/TA integration. Manual tenchniques were used for left obturator release prior to sidelying activity. Shoe wear was addressed to optimize her ankle/foot stability but care was taken due to her hip deformity, and we could not fully stablilize this for fear of this putting too much stress on her knee. Rhian continues to demonstrate weak left glute med with adductor integration in left stance, positive for left Trendelenberg. Her FAIR is quite good passively 35-40 degrees but still weak actively at 25 degrees. She is now working on right sidelying left inlet IPIR with balloon. She is also doing very well with a modified retro walk using two hands on the counter in order to perform it with support and lots of abdominals.”

Bilateral Knee Pain - Video of Before and After PRI Treatment

Mike Cantrell, MPT, PRC sent us a before and after video of a patient he has been working with over the last six months. 

“This patient was seen for treatment of bilateral knee pain and patellar tracking issues and one of our therapists and I began a lengthy program that put heavy emphasis on glute max work bilaterally.  This was a girl who had constant knee pain and wore pull-on knee braces all the time.  She progressed to a point that she is now able to perform athletics without knee pain and only wears the braces during athletics.  Nice success and a really good version of obvious change.” - Mike Cantrell, MPT, PRC

Thank you for all of the work you do.  You are great.  Also, the class was really enjoyable.  This stuff is addicting…I want to keep learning more.  Also, please tell Ron thank you again for all of the hard work that he does to teach and educate us all.  This has really changed the way I practice and came at a time I was becoming frustrated with limited outcomes and the PT profession.  It has certainly rejuvenated me.  So again, thanks for everything.

PEC Footwear Considerations - Follow Up

We received these QUESTIONS regarding the video and shoe recommendation for PEC:

Are you trying to pronate the feet?
How does this affect the extension tone?

ANSWER: When someone is in a PEC position their pelvis is forward, hips are in ER, and they are in a supinated position. The video was used to demonstrate that by allowing the patient to get in a more pronated position it assisted her to inhibit extensor tone and shift in her hips to be neutral.  Remember we don’t want our patients to be supinated or pronated…we want them to be somewhere in the middle during mid-stance phase of gait.  In the video, the patient was in a supinated position with the Asics Foundation due to weak glutes.  Allowing her to pronate more with a less supportive shoe allowed her to stay neutral in her pelvis.  She still needs glute work. Some PEC patients can overly pronate and have laxity with their feet too. - Lori Thomsen
