Updated PRI Vision Triage and New Self Assessment FormsWe have made a few updates to the PRI Vision Triage Forms! Please note that we’ve added instructions to follow when referring a patient to PRI Vision, as well as when other integrative disciplines are needed. We have also added a PRI Vision Self Assessment Form for those who are self-referring, as well as other disciplines (other than PTs/OTs) referring patients to PRI Vision.
