In our Pelvis Restoration course the speaker uses a female pelvis model with moveable SI joint throughout the course to demonstrate concepts of pelvic rotation, internal and external rotation. PRI has purchased several of these pelvis models from Sawbones, and labeled them according to the course concepts you will learn. Why did PRI establish a credentialing process for athletic trainers and strength and conditioning coaches?

The designation of Postural Restoration Trained (PRT) was established in order to:

1. Maintain continuity between sites implementing Postural Restoration Institute concepts and techniques.
2. Recognize individuals with PRI interest, specialization and expertise.
3. Protect the use and application of PRI science, reasoning, processes, techniques, and materials.
4. Provide avenues for professional development, collaboration between multidisciplinary specialists with PRI interests, and enhancement of scientific approaches using PRI concepts.
5. Allow educational institutions, students, and researchers access to PRI specialists.

Is there an application fee? Is there a cost to participate in the credentialing process?

While no application fee is required, a one-time fee of $2000 will be due prior to the one day credentialing process. This is the only monetary requirement and credentialing will not require renewal. This fee directly offsets costs associated with testing, assessing competency and completing the credentialing process. The fee will also assist us in developing the process, advancing individual knowledge of Postural Restoration Institute™ concepts and in growing a network of professional support. Those who earn the designation of PRT will receive Postural Restoration Institute™ course updates, ongoing discussion and dialogue, 50% discounted tuition to PRI courses (excludes Interdisciplinary Integration and PRI Vision Courses), advertising and promotional opportunities, and other benefits to be determined by the Postural Restoration Institute™. 

Is there a deadline for submitting an application?

Yes. Your complete application must be mailed or emailed to PRI by October 15th for testing the following January.

Am I eligible to apply?

You are eligible to apply if you are an athletic trainer with current certification by the Board of Certification (BOC). Verification of current certification status is needed (electronic or written). For more information about certification through the BOC, click here.

Please note that Impingement & Instability is offered immediately prior to PRT testing and you can apply for PRT indicating your intention to complete this course immediately prior to testing.

Is there a minimum number of years practicing PRI before I can apply for PRT credentials?

PRI strongly recommends experience and implementation of Postural Restoration concepts for 2-3 years before applying for PRT.

Does credentialing require renewal?

No. While we encourage and anticipate a high level of involvement from PRT individuals, status will not be affected by future Postural Restoration Institute™ support and involvement. Rather, in good faith we ask that you keep abreast of all Postural Restoration Institute™ activity and development. If other requirements are deemed appropriate in the future they will be determined with the involvement and support of the Postural Restoration Institute Faculty and Director of Credentialing.

When does credentialing take place?

If you choose to complete the PRT application by the October 15th application deadline and are encouraged to proceed through the process, you will be contacted between November 15th and December 1st. Testing will take place for two days following our Impingement & Instability course at the Postural Restoration Institute™  in Lincoln, Nebraska. Upcoming dates for PRT credentialing include January 8-9, 2012 and January 14-15, 2013. Testing is an educational and learning process that will include both clinical and analytical written examination.

The Postural Restoration Institute recently established a credentialing process for athletic trainers and strength and conditioning coaches as a way to recognize and identify individuals with advanced training, extraordinary interest and devotion to the science of postural adaptations, asymmetrical patterns and the influence of polyarticular chains of muscles on the human body as defined by the Postural Restoration Institute. Postural Restoration Trained (PRT) is a credential available to Athletic Trainers (with Certification through the BOC) and Strength and Conditioning Coaches (with CSCS Certification through the NSCA or SCCC Certification through the CSCCa) who have attended PRI courses, demonstrated a thorough understanding of the science through completion of the PRT Application, and successfully participated in both clinical and analytical testing.

If you are a physical or occupational therapist considering referring a patient to the PRI Vision Clinic, please fill out the PRI Vision Program Triage Forms below.  If you are a clinician from another discipline other than physical or occupational therapy (dentist, physician, optometrist, etc.), OR if you are self-referring, please fill out the PRI Vision Self Assessment Forms below.  Fax the completed forms to Kelly at the Hruska Clinic at 402-467-4580.  Once the PRI Vision Triage/Self Assessment Forms are received, Ron Hruska, PT and Dr. Heidi Wise, OD, FCOVD will review the information and determine if the patient is an appropriate candidate for the PRI Vision Clinic.  Kelly will then be in contact with the referring clinician/patient to give them further instructions on scheduling.  Please do not have your patient call to schedule an appointment unless they have already been contacted by Kelly.  Dr. Wise is typically in the PRI Vision Clinic on Tuesdays and Thursdays, therefore it may be a few days before you receive further information after faxing the PRI Vision Triage/Self Assessment Forms.  Thank you.

Q.  What is the PRI Vision Program?
A.  The PRI Vision Program is an interdisciplinary approach to achieving changes in body posture and patterns.  Vision has a tremendous influence in body posture and patterns; over 70% of all the connections in our brains relate to our vision, but only a small portion of these connections are concerned with how clearly we see.  A significant portion of nerve fibers that leave the retina go to the part of the brain that sends signals to the posturing muscles in our back.  The PRI Vision Program looks at this interaction and addresses how this can influence your PRI Physical Therapy Program both positively and negatively.  Just as a physical therapy program requires intermittent changes in activities to continue progress, the Vision Program is a part of the overall process and may require changes and adjustments to your treatment plan as progress is either made or not made at a satisfactory rate.

Q.  How is a prescription from a PRI Vision Optometrist different than one from another eye doctor?
A.  First, you will have your prescription checked while you are standing and with a trial frame on instead of having you look through two small holes in a machine while sitting.  This allows us to evaluate you in a more natural and dynamic position.  Secondly, we have found that in many cases the lens that the patient subjectively states makes their vision the most clear is not only different in a standing position than in a seated one, but also that it further “locks” their body into its usual posture or pattern.  This is what we are trying to change.  Only a PRI Physical Therapist working with a PRI Vision Optometrist can assess what prescription will help or hinder your PRI treatment program. 

Q.  Why is my vision more clear with my old glasses or contacts?
A.  Although how clearly you see is important, the purpose of your new prescription is primarily to force your brain to “use” your vision differently.  This often requires a change in your prescription that will not allow you to see the same as you did before.  During your evaluation, we frequently ask how you feel about how you are seeing; this is to gauge how easily you will adjust to your new Rx.  Some adjustment is necessary although each patient is different in how long that process will take. 

Q.  How long will it take for me to “get used to” my new glasses?
A.  Everyone is different.  Several factors will increase the time for adjustment, including an aggressive change in prescription from your current glasses, contact lens wear, lack of previous or prolonged time without eyeglass wear, poor compliance with prescribed wearing schedule, and bifocal lenses (either lined or unlined).  Keep in mind the PRI Vision Program is focused on facilitating changes for your long term overall well-being; although changing how you see may not be an easy adjustment, your physical therapist has assessed this to be a necessary process to achieve your physical goals. 
 
Q.  What if the glasses stop working?
A.  Just as you have likely experienced with your PRI program, PRI Vision is also a process.  In some cases, the effect you first experience with your glasses may seem to “wear off”.  This is due to a very natural process, called adaptation.  As humans, we try to avoid discomfort.  Changing to a new body posture or pattern isn’t comfortable, and your brain may learn to stop reacting to the new prescription in the desired way.  Often, your PRI physical therapist may have noted your ability to adapt as they have worked with you.  If so, this will be considered as changes in your eyewear prescription are trialed during your evaluation and the treatment plan is developed.  Ultimately, there is no way to know for sure which patients will experience this or how quickly it may happen.  If adaptation occurs, you will be re-evaluated to determine if eyewear is still needed as part of the treatment program. 

Q.  Will I always need glasses to continue feeling better? 
A.  In most cases, no.  As you continue working through your PRI program, the physical activities are reinforcing your new body posture or pattern, as well as your brain’s ability to adopt and maintain these changes.  The longer the incorrect posture or patterns have been present, the longer it will likely take to achieve and maintain the desired changes.  Our goal is to keep help patients learn and maintain their new pattern so well that it no longer matters if they have on a particular prescription. 

Q.  What if I don’t feel as good with my new glasses as I felt in the office during my evaluation?
A.  Many factors influence the effect your glasses will have.  The accuracy of the prescription, including the lens material and frame size, is the main one.  Additionally, a bifocal lens, whether lined or unlined, may also change how you feel and there is no way assess this prior to receiving your new eyewear.  Keeping your frame properly adjusted, especially the straightness of the frame as it sits on your face, is very important; a frame that sits crooked changes the prescription you are looking through and may hamper or even reverse your progress.  If you suspect there is a problem, you need to first have your prescription verified for accuracy.  Once your prescription has been verified, schedule an appointment with your PRI physical therapist so they may evaluate you with your new eyewear to determine whether re-evaluation by the PRI Vision Optometrist is needed. 

Q.  Why do I need to be seen again by the PRI Vision Optometrist? 
A.  There are a few main reasons:
If you wore glasses before starting the PRI Vision Program to see more clearly, you will need to be re-evaluated to determine your final prescription as you discharge active treatment.  This is to ensure that you are able to maintain the changes you have made in your posture and position regardless of what prescription you have in your glasses. 
If the effect of the PRI Vision Program eyewear prescription is no longer evident, which may mean adaptation has occurred, or you are not progressing in your PRI physical therapy program as expected and your physical therapist suspects your vision is continuing to influence this.
Generally, all patients will be seen for at least one follow-up to gauge whether the vision component has been adequately addressed and to recheck the clarity of your vision. 

Q.  Do I need to still see my regular eye doctor?
A.  Yes.  Regular ocular health evaluations are recommended to check for eye diseases such as glaucoma and macular degeneration.  Most eye diseases do not affect how clearly you see until you have had them for a long time.  This is not part of your PRI Vision Evaluation.  We ask that you do not fill any eyeglass prescriptions from another eye doctor while undergoing your PRI Vision Program.

Ciało ludzkie jest zbalansowane poprzez integrację systemów, które nie są w równowadze. Na przykład tułów, jest w pełnej równowadze z wątrobą położoną po prawej stronie i sercem po lewej. Dominacja kończyny jest równoważona poprzez ruch naprzemienny; ruch lewej kończyny górnej z dolną prawą w czasie chodu i na zmianę.

Terapeuci szkoleni przez PRI są świadomi tych właściwości ciała ludzkiego i typowych wzorców zwiazanych z osłabieniem czynności-funkcji systemów, których przyczyną jest nadużywanie dominujących wzorców. Przyczyną nadużywania jednej strony ciała mogą być asymetryczne przeciążenie funkcjonalne innych systemów ciała. Np. jeżeli mniejsza, lewa przepona nie jest efektywna w procesie oddychania w takim stopniu jak prawa, doprowadzi to do skręcenia tułowia a co za tym następuje całego ciała. Prawa przepona, większa niż lewa, jest zawsze w lepszej pozycji funkcjonalnej ze względu na podparcie strukturalne ze strony wątroby. Dlatego funkcja mięśni brzucha lewej strony jest tak ważna dla równowagi tułowia w czasie ruchów naprzemiennych takich jak np. chód.  Utrzymanie funkcjonalności prawej części klatki piersiowej(jej „otwarcia”) w procesie oddychania jest zawsze bardzo trudne ze względu na brak mięśnia sercowego po prawej stronie. Pozycja stania na prawej nodze w celu skompensowania ciężaru lewej górnej klatki piersiowej, wspomaga przesunięcie lewej miednicy do przodu i prawego kompleksu barkowego do dołu. Ta asymetryczność uzupełnia szczegółowe funkcje dwóch półkul mózgowych. Chociaż obydwie półkule wspóldziałają w pewnych funkcjach organizmu, jednak każda ma swoją „specjalność”. Każda półkula kontroluje przeciwną stronę ciała. Lewa strona mózgu związana jest z mową i dlatego prawa kończyna górna pełni dominującą funkcję w komunikacji, rozwoju i wzroście ciała. Terapeuci szkoleni przez PRI są w stanie rozpoznać, kiedy ten naturalny wzorzec nie jest dostatecznie zbalansowany poprzez neurlogiczną i mięśniową aktywność lewej kończyny.

W momencie kiedy te normalne „zaburzenia” nie są regulowane przez naprzemienny ruch w czasie chodu, oddychania i skrętów ciała, wyłania się silny wzorzec powodując osłabienia, niestabilności i syndromy bólowe systemu mięśniowo –szkieletowego. Zrównoważenie aktywności mięśniowej okolicy k. krzyżowej ( miednica), mostka (tułów) i k. klinowej(część środkowa czaszki) porzez leczenie konceptem PRI najlepiej odbudowuje funkcję  systemów ciała ludzkiego. Wszyscy terapeuci szkoleni przez instytut PRI potrafią użyć naprzemienną funkcję w celu zmniejszenia dominacji lewej miednicy i prawej kończyny górnej oraz funkcji oddechowej ciała w celu osiągnięcia maksymalnego przepływu powietrza z i do prawego płuca.

Wzrok, wymagania zawodowe, pozycja w łonie matki, itp, mogą wpłynąć na asymetryczne tendencje i wzorce ciała. Wieloryby na dnie oceanów odżywiaja się na prawym boku, lemury w większości sa leworęczne, ropuchy używają prawej łapy częściej niż lewej, szympansy trzymają gałęzie w lewej dłoni i chwytają owoce prawą,  a ludzie zwykle utrzymują środek ciężkości na prawej nodze dla lepszej równowagi i ze względu na łatwość osiągnięcia tej pozycji. Terapeuci szkoleni przez Instytut są w stanie rozpoznać zintegrowane wzorce ludzkiej postawy, używania kończyn, funkcji oddechowej, zaburzenia równowagi, orientacji żuchwy i dynamiki stopy; i zrównoważyć te wzorce w maksymalnym stopniu poprzez specyficzny program ćwiczeń, który integruje funkcjonalne, poprawne procesy oddychania lewej strony ciała poprzez inhibicję prawej lub jej współdzialanie.

Dr. Kim and Dr. Chang of Kim and Chang Spine and Chiropractic Clinic hosted a three day course covering both Myokinematic Restoration and Postural Respiration.

Over 20 attendees were present at this course.  The majority of the class didn’t speak English, however, Dr. Kim was there to translate. 

By the end of the three days, Ron had all the attendees checking Adduction Drop Tests, performing 90-90 Hemibridges, and performing a manual Left AIC technique!

While not teaching, Ron was able to learn about Dr. Kim and Dr. Chang’s practice and their current method of treatment.  The majority of the physicians present were Oriental Medicine Doctor’s.  While there, Ron received two vitamin injections, which he said “were excellent”.  Below is a picture of Ron with two Doctors and behind them is the area where the herbs are stored.

History of Passion

As the Postural Restoration Institute (PRI) approaches the end of its first decade the staff of PRI, Postural Restoration Certified (PRC) therapists, and I, are often asked about the history of PRI. It’s an Institute that is truly built around 30 years of clinical practice associated with re-occurring successes of specific patient treatment programs. Consistent evidence-based correlations, discovered with patient biomechanical, respiratory and neurological functional patterns, as well as predictable functional limitations, allowed us to establish reproducible outcome based programs.

My passion for cranio-dontics, the respiratory system’s influence on neuro-mechanics, bi-pedalism, and pathokinesiologic patterns of tri-planar organization and muscle integration, autonomic nervous system influences on the stomatognathic and somatosensory systems, neuro-optometric effects on the vestibular system and multi-sensory control of posture relating to primitive and developmental reflexes, has resulted in an examination process of the asymmetrical human body and an intervention process called Postural Restoration.

Asymmetrical Function

My clinical orientation and methodology of intervention has and will always be guided by practical experience and a predictable, objective outcome approach rather than precepts or theory, since the complexity of diagnostic classification, complexity of diagnostic language and labeling, and the complexity of professional consensus regarding treatment intervention exists across many, if not all, health care professions. We live in an evidence based society that still believes the left appendages of our musculoskeletal system are in the same symmetrical position as the right, or vice versa. Research methodology, evaluation guidelines and treatment intervention do not include steps or measures that need to be taken into consideration because of our asymmetrical architecture, fluid-dynamics, gaseous diffusion and cerebral function.

The pre-dental and pre-medical classes, as well as the dental and medical program classes, that I attended were not taught with text-books that differentiate the uniqueness of a right lateral pterygoid muscle of the face, from it’s counterpart on the left, because of typical human organization of cranial torsion, sidebending and sagittal rest in the right dominate oriented human.

I was not informed about the unique qualities of the human roborant left hamstring, left femoral adductors, left gluteus medius, left diaphragm leaflet, right gluteus maximus, left inferior oblique of the eye, left subclavius, left transverse abdominis and triangularis sterni, and the right peroneals as a family of muscle that is needed to counteract forces produced by their counterparts in the typical asymmetrical pattern of the upright human.

Hypertonicity

Overuse of anti-gravitational muscle on one side leads to C1 C2 mal-alignment, dental malocclusions, facial, cervical, thoracic, lumbopelvic, acetabular–femoral, femoral-tibial, etc…torque, scoliosis, cervical hypolordosis, lumbar hyperlordosis, and most importantly system hypertonicity.

Recognition of Altered Systemic Muscle

These neuro-mechanical issues can be initiated or compounded by visual and vestibular paradoxical function, tooth and mandibular functional limitations, respiratory patterns of dysfunction or pulmonary dysfunction, and improper foot proprioceptive feedback, leading to altered force and timing of systemic muscle contraction. All of the above associations can now be included and integrated in the design of a treatment program that uses PRI algorithms.

There is, has been, and always will be strong evidence that supports the predictability of human posture and the ability to manage human intrinsic forces associated with human primitive and developed reflexes. Typical or normal imbalances of muscle strength, normal muscle length discrepancies associated with visible typical pelvic torso and facial torsion, standard human patterns of strategic compensation, and modeled patterns of neuromuscular anxiety, however, is not referenced or outlined in current literature.

Polyarticular Muscular Chains

The human develops strong usage of his or her left Anterior Interior Chain (Left AIC), right Brachial Chain (Right BC), right Temporal Mandibular Cervical Chain (Right TMCC) and Posterior Exterior Chain (PEC) early. Consistent patterns of cervical torticollis, scoliosis, femoral-patellar compression and cranial-thoracic asymmetries are a few examples of early-adaptation to gravity and human asymmetry. Naming these chains seemed so appropriate and empirically easy. No one, nationally or inter-nationally had ever looked at or addressed this issue of human integrated asymmetry or put together physically, physiologically, or psychologically issues at multi levels or with multi systems, in the manner PRI does.

Synergistic Mentors

I’ll never forget, as a former dental student, who dropped out of the dental tract to pursue reasoning behind teeth being chewed up on one side, the impact Brunnstrom, Knott and Voss had on me in my early stages of didactic understanding of synergistic patterns of function. However, Gindler and Buckholz empowered me, through their European philosophy of hyperinflation association with hypertonicity early on in my career of physical therapy. Janda, Chaitow, Gracovetsky, Borch-Jacobsen and Myers were never mentioned or referred to at any of the myriad of post-graduate courses I took regarding myofascial release, mobilization, cranial-sacral, muscle energy, strain- counterstrain, etc. I never fully gravitated toward these post-graduate concepts because of their familiar “syndrome” or “symptom” approach and application.

Interdisciplinary Integrative Cause

For every effect there is a cause, and the question of ‘cause’ motivates my passion for integrative thought and reason. I will always be appreciative of individuals, including and like, Zink or Pope, ‘The Common Compensatory Pattern’, Previc, ‘Cerebral Lateralization’, Mezieres, ‘The Mezieres Method’, Isogai, ‘Dynamic Therapy’, Fonder, ‘The Dental Physician’, Schomberger, ‘The Malalignment Syndrome’, James and Stokon, ‘Cranial Strains and Malocclusions, Padula, ‘Visual Midline Shift’, Magoun or Gehin, ‘Cranial Osteopathy’, and Rubin, ‘Voice as Related to Handedness’, because of their search for the integrative cause.

Hemispheric Plasticity

We all appreciate the fundamental differences in the neural organization of the left and right cerebral hemispheres. My first real orientation to plasticity of the motor cortex occurred while working with patients who had cerebral vascular accidents. It was always more difficult to establish a center of gravity with the right lower extremity with a patient that lost the Broca and Wernicke area of the left hemisphere, than on a person with a right cerebral vascular accident. As I worked with non-cerebral vascular accident amputees in the same practice setting I saw the same trend. Limited left hip extension, adduction and stability with left lateralization, limited left thoracic abduction and left abdominal facilitation exists to some degree in all of my patients when compared to the right side regardless of neuromuscular or orthopedic etiology. In essence it was actually easier to functionally rehabilitate the right cerebral vascular accident patient than the left lower extremity amputee, because of left cerebral plasticity and difficulty in reducing left lower motor hip flexor and abductor tone on the left lower extremity amputee.

Regulatory Functional Lateralization

Functional lateralization, fundamental differences in the neural organization of the left and right cerebral hemispheres and cyto-architectural anatomic brain asymmetry was also recognized in the patients I worked with at a hospital outpatient based physical therapy department. Young women especially, who presented difficulty with left lower extremity adduction and experienced right knee pain, did immediately better mechanically and symptom wise, using the same left extension, adduction and hip internal rotation program I used on my patients who had amputations, cerebral vascular accidents or chronic obstructive pulmonary disease. Patients with anterior knee pain, regardless of the side, who went through countless hours of rehabilitation with little success, had a reduction of pain immediately with the establishment of left acetabular femoral and femoral acetabular internal rotation as they had on the contralateral side. I knew from these types of experiences that the left pelvis and lower thorax was consistently forward, unstable, more often anteriorly rotated, and more restrictive than the right. The right cerebral vascular accident patient needed left hamstrings for return of reciprocal gait, keeping in mind he or she probably didn’t have full extension or adduction on the left prior to his or her cerebral accident. The left lower extremity amputee with similar gait issues needed a left hip flexor inhibition program. Anterior knee pain patients needed the same rehabilitation approach on the left side as the above patients, regardless of the side their knee pain existed.

Unilateral Compensatory Patterned Function

The unilateral orientation of organs and the vagal system, unilateral diaphragmatic function, unilateral neglect, unilateral lymphatic emptying, and unilateral binocular dominance all contribute to predictable, measurable and ineluctable patterns of vestibular-ocular, cranio-mandibular, cervical-thoracic, thoracic-lumbar, lumbo-pelvic, pelvic-femoral, femoral-tibial, tibial-calcaneal and rear foot-fore foot upright, synergistic, reflexive and non-reflexive compensatory function.

Multifaceted Approach of Non-Linear Treatment

To reduce the nature of unilateral influences on compensatory neuro-mechanics or the possible compensatory neuro-mechanical influences on our unilateralization; empiracle experiences and a balanced, cross-linked mind-set is needed. The day after I graduated from physical therapy school, I started classes in public administration to balance my years of biology with business, economics, human-resource development, gerontology, public and private organization classes, etc…and quickly realized that success in the clinic depends on multifaceted, integrative and intuitive approaches. The Omaha Veterans Administration Medical Center and their rehabilitation process in the seventies and eighties allowed me to integrate closely with speech therapists, orthotic and prosthetic specialists, physiatry based neurologists, occupational therapists, and social workers who empowered the most handicapped to levels that no sole specialist would ever achieve. I soon realized balancing a budget using zero-based methodology or balancing muscle around a proximal joint requires input from many sources that stimulate or inhibit specific measures for successful outcomes and self-regulation. Our bodies are far from being in equilibrium. They may be stable but they can only maintain this stability by a constant ongoing process of adjustment and self-regulation. Prigogine, a Nobel Prize winning physicist, described organisms as “non-linear, complex, dynamic, self-regulating systems that are far from equilibrium”.

Imbalanced Inhibition vs. Balanced Strengthening

The capacity for self-healing and self-regulation provides me the basis for new paradigms. For example, the ventilatory process was never made clearer to me than when I worked in a burn unit. My respect for the diaphragm, it’s dermis healing abilities, and it’s influence on the lumbo-pelvic-femoral complex would not have been realized if I did not take advantage of opportunities that existed in a hospital setting. As a director of the physical and occupational therapy department, I had achieved the ability to reduce use of isokinetics for functional rehabilitation and increase the use of hamstring, gluteal, adductor, abdominal and hemi-diaphragm unilateral postural oriented programs. I developed a system that isolated left hamstrings, independent of speed, gravity or position to balance the pelvic femoral sagittal forces and pelvic biomechanical planes. Protonics® Neuromuscular Repositioning allowed me to finally maintain a neutral lumbo-pelvic position during low back or lower extremity retraining through hip flexor inhibition. The quintessence of health is an active, on-going attempt to inhibit imbalancing force. Every time I see a teeter-totter on a playground I’m reminded of this essential element. The body is full of active teeter-totters constantly being challenged with purposeful asymmetries and dynamic imbalances.

PRI Guidance

After 16 years of hospital and outpatient based care, I knew I had reached a climacteric in my life, when a majority of my patients with temporo-mandibular dysfunction, shoulder impingement, cervicalgia, low back strain, epicondylitis, anterior cruciate ligament repair, pelvic floor dysfunction, etc., were reaching optimal functional outcomes through the incorporation of left acetabular femoral internal rotation, the isolated facilitation of left femoral adductors and left thoracic abductors, the establishment of a left zone of apposition and the enhancement of right brachial expansion. It was often difficult to convince physicians and physical therapists to reposition a pelvis and a femur before stretching out an iliotibial band or performing an iliotibial band release, surgically. Because of these common rehabilitation measures and other aforementioned issues I developed a private practice and established the Postural Restoration Institute®. My clinical experiences and outcomes and my former and present patients continue to guide PRI educational endeavors.

Unorthodox Gratification

The gratification on a patient’s face or tears that follow the manual application of a PRI right subclavius technique to restore full right humeral glenoid internal rotation or left cervical rotation or to reduce thoracic outlet symptoms, reinforces the need for assiduous attention given to the development, formation and application of each PRI manual or non-manual technique. The heterodox beliefs of PRI application and Left AIC, Right BC and Right TMCC principles are usually quickly accepted because of the objective, consistent success of reducing tri-planar pattern restrictions and multi-level symptomatic pain or discomfort.

PRI Neuromuscular Orthotic Principles

Acceptance of PRI theory allows the therapist to use PRI neuromuscular orthotic principles to reduce right femoral adductor tone and improve left lateralization with right medial longitudinal arch contact, or bi-nasal optical occlusion to improve mid-brain, ambient, parasympathetic activity, or an elastic functional abdominal support across the left abdominal region to help maintain a left zone of apposition, or a maxillary sagittal or pivotal oral appliance to reduce internal rotation of the right temporal bone, or motion controlled footwear with calcaneal stability to reduce TMJ compression or clenching, or first metatarsal head proprioceptive contact on the right side to improve arm swing, or gait retraining through left arm-right leg lead to reduce right sacral-iliac joint dysfunction, etc., with other complimentary non-manual and manual PRI techniques.

PRI Mission

PRI now has 47 Postural Restoration Certified (PRC) physical therapists, 24 PRC centers, 5 educational courses (Myokinematic Restoration, Postural Respiration, Cervical-Cranio-Mandibular Restoration, Impingement and Instability, Advanced Integration and Interdisciplinary Integration.  PRI has produced three editions of a CD-Rom comprised of Non-Manual Techniques and most recently finished the first edition of a Manual Techniques DVD.  PRI’s website (posturalrestoration.com) continues to be the number one resource for information regarding courses, location of nationwide PRI health professionals, educational materials, and references.   Most recently, the website has developed into a dynamic resource providing its audience with daily news and dialogue direct from PRI.  PRI concepts continue to grow both nationally and internationally.  The PRI mission is to explore postural adaptations and asymmetries and their influence on the polyarticular chains of the human body.  Our mission is based on the development of an innovative treatment approach that explains the primary contributors of postural kinetic and kinematic movement dysfunction. 

Commitment to Lifelong Learning

Reading journal articles, books, research and curricular material from other professions, almost on a daily basis, has increased my voracious appetite and my insatiable curiosity for information on anatomical and physiologic imbalance. My pedagogic mind is a reflection of my commitment to test knowledge through experience, persistence and willingness to learn from mistakes. The Anterior Interior Chain of our bodies is a reflection of pedagogic reasoning, leading to a PRI experience. By continuing to commence with PRI experience in aquatics and PRISM pool design and application, with optometrists and vision retraining, with dentists and cranial alignment and with my patients and their sincere desire for guidance, I will continue to connect links.

Commitment to Service

My mindset for non-linear linkage existed long before serving on the Model Practice Act Task Force of the Federation of State Boards of Physical Therapy, or serving as the youngest president of the Nebraska Physical Therapy Association, or serving on the American Physical Therapy Associations reference committee and judicial committee, or serving on the State Board of Examiners in Physical Therapy. As Leonardo da Vinci recognized and appreciated “interconnectedness of all things and phenomena”, so do I. Long before the Guide to American Physical Therapist Practice was written, I embraced ambiguity, paradox and uncertainty with a passion. Passion for PRI application, education and integration reduces the insecurities and instabilities of those with the same embrace.

Posture is a reflection of the “position” of many systems that are regulated, determined and created through limited functional patterns. These patterns reflect our ability and inability to breathe, rotate, and rest, symmetrically with the left and right hemispheres of our axial structure.

“Limited functional patterns” refers to movement that is restricted in directions, planes or normal boundaries of functional range, as a result of improper joint, muscle, and mediastinum rest position. Function is therefore limited because soft tissue and osseous restrictions prevent one from using muscles and joints in their normal range. Adaptation and compensation for these limitations require neuromotor encoding and hyperactivity of muscle that is placed in improper positions that exceed normal physiological length, or in positions that make them a mover or counter-mover in planes and directions that are not observed when one is in a neutral or more symmetrical state of rest. This compensatory activity and hyperactivity usually becomes dysynchronous in the accessory muscles of respiration and at the appendicular flexors and axial extensors, thus limiting functional rotation at the trunk and through the lumbo-pelvic-femoral and cranial-mandibular-cervical complex.

Postawa jest odzwierciedleniem powiązań pozycji pomiędzy wieloma systemami organizmu.  Są one tworzone i regulowane poprzez ograniczające wzorce funkcjonalne. Wzorce te, symetrycznie w prawej i lewej części ciała, związane z oddychaniem, trójpłaszczyznowym ruchem i pozycją końcową kreują możliwości i restrykcje.

Ograniczające wzorce funkcjonalne odnoszą się do ruchu, który jest zablokowany w zakresie, płaszczyźnie i kierunku w wyniku nieprawidłowej pozycji spoczynkowej mięśni, stawów i śródpiersia. Funkcja jest zatem limitowana przez układ kostny i tkanki miękkie uniemożliwiając wykonanie prawidłowego ruchu. Adaptacje i kompensacje dla powyższych limitacji wymuszają: nowe neuromotoryczne kodowanie, hyperaktywność mięśni, które znajdują się w nieprawidłowej pozycji powodując nadmierne niefizologiczne   rozciągnięcie, lub błędną rekrutację jako agonistów/antagonistów, zmuszając je do pracy w nietypowych kierunkach i płaszczyznach, nie występujących w neutralnej lub zbliżonej symetrycznie pozycji. Ta kompensacja i hyperaktywność mięśni zazwyczaj staje się dysynchronizmem dla dodatkowych mięśni oddechowych, zginaczy kończyn i osiowych prostowników ograniczając funkcjonalną rotację tułowia, jak i zespoły lędźwiowo-miedniczno-udowy oraz czaszkowo-żuchowo-szyjny.

PRI established a certification process in 2004 as a way to recognize and identify those individuals with advanced training, extraordinary interest and devotion to the science of postural adaptations, asymmetrical patterns and the influence of polyarticular chains of muscles on the human body as defined by the Postural Restoration Institute. Certification is available to physical therapists, physical therapist assistants, and occupational therapists who have attended PRI courses, demonstrated a thorough understanding of the science through completion of the PRC application, and successfully participate in both clinical and analytical testing. Why did PRI establish a certification process?

Certification was established in order to:

1. Establish and maintain continuity between clinical sites in areas of research, practice and student affiliation.
2. Recognize individuals with PRI interest, specialization and expertise.
3. Protect the use and application of PRI science, reasoning, processes, techniques, and materials.
4. Provide avenues for professional development, collaboration between multidisciplinary specialists with PRI interests, and enhancement of scientific approaches using PRI concepts.

Is there an application fee? Is there a cost to participate in the certification process?

While no application fees are required, a one-time certification fee of $2000 will be due prior to the 2-day certification process. This is the only monetary requirement and certification will not require renewal. This fee directly offsets costs associated with testing, assessing competency and completing certification. The fee will also assist us in developing the process, advancing individual knowledge of Postural Restoration Institute™ concepts and in growing a network of professional support. Is there a deadline for submitting an application?

Yes. Your complete application must be mailed to PRI by September 15th for certification the same year.

Am I eligible to apply for certification?

You are eligible to apply if you are a physical therapist, physical therapist assistant or occupational therapist who has completed the following PRI sponsored courses: Myokinematic Restoration, Postural Respiration, Cervical-Cranio-Mandibular Restoration, and Advanced Integration.

I attended an APTA state sponsored PRI course. Does this attendance fulfill the course attendance requirement for applying for PRC?

No. PRI does not have access to attendance records outside PRI sponsored courses and agendas are often modified per sponsor’s time schedule. Therefore, courses must be sponsored by PRI and therefore presented by PRI Faculty or Associate Faculty using PRI materials.

Course must be a full 15 contact hours in length for Myokinematic Restoration, Postural Respiration and Cervical-Cranio-Mandibular Restoration. Course must be a full 30 contact hours in length for Advanced Integration.

Is there a minimum number of years practicing PRI before I can apply for certification?

PRI strongly recommends clinical experience and implementation of Postural Restoration concepts for 2-3 years before applying for PRC.

Does certification require renewal?

No. While we encourage and anticipate a high level of involvement from certified individuals, certification status will not be affected by future Postural Restoration Institute™ support and involvement. Rather, in good faith we ask that certified individuals keep abreast of all Postural Restoration Institute™ activity and development. If other requirements are deemed appropriate in the future they will be determined only with the involvement and support of the Postural Restoration Institute Faculty and Director of Certification.

When does certification take place?

If you choose to complete the certification application by the September 15th application deadline and are encouraged to proceed through the process, you will be contacted between October 15th and November 1st. Certification will take place on Monday and Tuesday following our annual Advanced Integration course at the Postural Restoration Institute™  in Lincoln, Nebraska. Testing is an educational and learning process that will include both clinical and analytical written examination.

Once received, your application will be reviewed by our Application Review Committee and you will be notified of their feedback and comments. If you choose to complete the process after consideration of all reviewer’s feedback, certification of both clinical and analytical testing will take place for two days following our Advanced Integration course in December.

While no application fees are required, a one-time certification fee of $2000 will be due prior to the 2-day certification process. This is the only monetary requirement and certification will not require renewal. This fee directly offsets costs associated with testing, assessing competency and completing certification. The fee will also assist us in developing the process, advancing individual knowledge of Postural Restoration Institute™ concepts and in growing a network of professional support. The Postural Restoration Institute established a certification process in 2004 as a way to recognize and identify those individuals with advanced training, extraordinary interest and devotion to the science of postural adaptations, asymmetrical patterns and the influence of polyarticular chains of muscles on the human body as defined by the Postural Restoration Institute. Certification is available to physical therapists, physical therapist assistants, and occupational therapists who have attended PRI courses (click here for course requirements), demonstrated a thorough understanding of the science through completion of the PRC application, and successfully participate in both clinical and analytical testing. To date 80 Therapists have earned the designation of Postural Restoration Certified (PRC) under the direction of Ron Hruska.

PRI Associate Faculty have contributed the following list of suggested PRI related research topics for students and researchers:

1. Determine prevalence of right sacro-iliac joint dysfunction with the following examination findings: positive Left Adduction Drop Test and negative Left Extension Drop Test.

2. Determine prevalence of anterior superior labral tears on the left vs. right side.

3. Study concomitant activation of the left adductor to oppose the right gluteus maximus.

4. Describe gait patterns and identify common trends between the different PRI classifications: Left AIC patho-compensatory pattern and non patho-compensatory pattern and PEC pattern.

5. Study data of seated, passive, hip internal rotation and the relationship to laxity of the Y ligament.

6. Record spirometer reading and EMG study of oblique and back extensor activity (inhale/exhale) to describe the difference between sitting with lumbar extension (via lumbar roll) and sitting with a neutral sacrum.

7. Study EMG activity when performing Hruska Adduction Lift Test and Hruska Abduction Lift Test.

8. Study standing A-P x-rays to determine validity and reliability of Adduction Drop Test for assessing pelvic position.

9. Study diagnosed labral tear patients to determine categories or patterns of right verses left.

10. Compare labral tear classifications using seated hip range of motion, Adduction Drop Test, Extension Drop Test, and x-rays to determine predictors of causes (ie. laxity, compression).

11. Study valid and reliable methods of documenting hip shifting and normal values.

12. Study axial influences on appendicular rest position.

13. Study the coorelation of Postural Restoration methodologies with lumbar canal opening and erect postural position.

14. The role of the hamstrings with PRI Prone Alternating Reciprocal Hamstring Curls.

15. The influence of apical chest wall limitations on lower extremity frontal plane activity.

Postural Restoration Institute™ techniques provide a conservative approach to the management of scoliosis. While techniques may not correct a spinal curve, significant gains can be made from reducing torsion and correcting mechanics of the rib cage.

Clinical experience of Postural Restoration Certified therapists provides specific clinical data to support the use of PRI techniques as much as if not more so than other approaches to physical therapy.

Ideally, postural restoration techniques would be initiated on curves between 0-25 degrees and on patients between the ages of 7-15 years of age. Treatment can effectively stop a curve from progressing and possibly reverse curvature when patient and parent compliance and understanding are at high levels. Another component of successful intervention requires a thorough understanding and implementation of integration techniques by both the therapist and patient. Early intervention is best! Adolescent curvature and rotational curvature is more difficult to manage after the age of 17.

Curves between 25-40 degrees may also be physician managed through the use of body jackets. Determining the necessity of a body jacket or bracing is a decision that is most suited for the physician and patient/parents. Whether it be a night or day body jacket, postural restoration can be simultaneously carried out. It is most critical at this stage that patient compliance is high as techniques will most likely need to be carried out three times per day. A night jacket with PRI activities during the day may be a good option for appropriate candidates for whom a physician has recommended a body jacket. The use of a jacket with postural restoration techniques can be a positive intervention and can assist the patient’s tolerance to the jacket. A physician may opt for surgical intervention when a curve is greater than 40 degrees.

At this time we have no supportive literature/research articles, pre-post x-rays, or case studies to support PRI intervention with scoliosis patients. We are still in the discovery stage of clinical data collection. Clinical outcomes, however, are very promising and positive especially when parents, patients, and physicians are all supportive of the PRI approach.

Instability at the base of the spine (L5-S1, L4-L5) also called a pathological curve vs. a non-pathological curve occurring further up the spine resulting from respiratory imbalance must be determined in order for proper treatment to be outlined using Postural Restoration Institute™ manual and non-manual techniques.

The large majority of diagnosed scoliosis (80-90%) coincides with a right thoracic curve. X-rays and/or physician diagnosis are needed to verify the level and degree of curvature. In addition, corresponding PRI tests of trunk rotation, extremity orientation, functional measurements of respiratory function, frontal, sagittal, and transverse plane activity all can provide the practitioner with important diagnostic information regarding the degree of instability and limitation that exist.

For those patients demonstrating instability at the base of the spine and therefore presenting with a pathological scoliosis, the need to achieve left acetabular-femoral internal rotation or proper position for stability of the hips is their primary treatment goal. Neutrality and stability of the pelvis can be achieved through Postural Restoration Institute™ non-manual techniques which activate the ipsilateral hamstrings and ischiocondylar adductor and contralateral gluteus maximus for sacral stability and neutral position. Once stability is gained, a proper tri-planar program can be initiated including dynamic trunk rotation for functional activities that will reduce muscle tone associated with the original muscular skeletal pattern of torsion.

For those patients demonstrating curvature without instability at the base of the spine (ie. non-pathological curve) the primary treatment goal is achieving apical expansion of the right upper chest wall and a strong oppositional foundation for left diaphragmatic contraction. Assisting proper air flow into the right anterior chest wall will provide more symmetry and balance to the trunk. PRI manual techniques for the right brachial chain will assist expansion and flexibility of the chest wall while PRI non-manual techniques will provide needed structural stability and support.

Posture is a reflection of the “position” of many systems that are regulated, determined and created through limited functional patterns. These patterns reflect our ability and inability to breathe, rotate, and rest, symmetrically with the left and right hemispheres of our axial structure.

“Limited functional patterns” refers to movement that is restricted in directions, planes or normal boundaries of functional range, as a result of improper joint, muscle, and mediastinum rest position. Function is therefore limited because soft tissue and osseous restrictions prevent one from using muscles and joints in their normal range. Adaptation and compensation for these limitations require neuromotor encoding and hyperactivity of muscle that is placed in improper positions that exceed normal physiological length, or in positions that make them a mover or counter-mover in planes and directions that are not observed when one is in a neutral or more symmetrical state of rest. This compensatory activity and hyperactivity usually becomes dysynchronous in the accessory muscles of respiration and at the appendicular flexors and axial extensors, thus limiting functional rotation at the trunk and through the lumbo-pelvic-femoral and cranial-mandibular-cervical complex.

Physical Therapists and PT Assistants

The American Physical Therapy Association (APTA) as a national organization does not approve continuing education providers or courses. Rather, state chapters create their own guidelines for course approval. Many states require that PT’s and PTA’s complete a certain number of continuing education hours in order to retain their licenses; but not all of those states require that the courses themselves be approved. In the states that require approval of CE courses for PT’s and PTA’s and where PRI is hosting a live course PRI courses are approved yearly. PRI has never been denied for continuing education credit.
PT’s and PTA’s are eligible to apply for Postural Restoration Certification (PRC).

Occupational Therapists and OT Assistants

The American Occupational Therapy Association (AOTA) recognizes PRI as an Approved Provider of continuing education. AOTA Provider #6596. Approval applies to PRI live courses. Please note that the assignment of AOTA CEUs does not imply endorsement of specific course content, products, or clinical procedures by AOTA. OT’s are eligible to apply for Postural Restoration Certification (PRC).

Athletic Trainers

PRI is recognized by the Board of Certification, Inc (BOC) to offer continuing education for certified athletic trainers (#P2376). 
ATs with Certification through the BOC are eligible to apply for Postural Restoration Trained (PRT).

Strength & Conditioning Coaches

The National Strength and Conditioning Association (NSCA) has approved the following courses for 2012: Myokinematic Restoration, Postural Respiration, Pelvis Restoration, Impingement & Instability and Advanced Integration. These are the PRI courses most beneficial to those designing strength and conditioning programs. Strength Coaches with CSCS Certification through the NSCA or SCCC Certification through the CSCCa are eligible to apply for Postural Restoration Trained (PRT).

Massage Therapists

PRI is approved by the National Certification Board for Therapeutic Massage & Bodywork (NCBTMB) as a continuing education Approved Provider. Provider number 451877-12. Approval applies to live courses only. 

Other Healthcare Professionals

PRI welcomes any interested healthcare professional to attend our courses. Attendees are responsible for following their state statutes regulating their professional practice as they integrate interdisciplinary PRI concepts. Certificates of Completion are awarded to attendees upon the successful completion of each course.
